Paravalvular leaks (PVLs) are a common complication of cardiac valve surgery, occurring in up to 5% of cases. PVLs can lead to significant morbidity and mortality, including heart failure, stroke, and endocarditis. Transcatheter paravalvular leak closure (TPVLC) is a minimally invasive procedure that can be used to treat PVLs without the need for open heart surgery.

TPVLC Techniques

There are a variety of TPVLC techniques available, each with its own advantages and disadvantages. The most common TPVLC techniques include:

  • Amplatzer Paravalvular Leak Occluder (PFO): The PFO is a self-expanding device that is placed across the PVL to seal the leak. The PFO is available in a variety of sizes and shapes to accommodate different PVL anatomies.
  • Edwards PASCAL Paravalvular Leak Closure System: The PASCAL system is a balloon-expandable device that is placed across the PVL to seal the leak. The PASCAL system is available in a variety of sizes and shapes to accommodate different PVL anatomies.
  • CardiAQValve Paravalvular Leak Closure System: The CardiAQValve system is a self-expanding device that is placed across the PVL to seal the leak. The CardiAQValve system is available in a variety of sizes and shapes to accommodate different PVL anatomies.

TPVLC Outcomes

TPVLC is a safe and effective procedure for the treatment of PVLs. The vast majority of patients who undergo TPVLC experience significant improvement in their symptoms and quality of life. The long-term outcomes of TPVLC are generally good, with most patients remaining free of PVL recurrence.

TPVLC is a valuable tool for the treatment of PVLs. This minimally invasive procedure can be used to effectively seal PVLs and improve patient outcomes. As the technology and techniques for TPVLC continue to evolve, it is likely that this procedure will become increasingly common in the management of PVLs.
